Unless your insurance has been pre-approved or you have made prior arrangements, payment is due at the time services are rendered. Accepted forms of payment include cash or check.

Mr. Harman is an in-network provider for Blue Cross Blue Shield and many other insurance companies and EAP's (Employee Assistance Programs). Fees are accepted in accordance with these insurance plans and must be pre-approved.

Please be aware that not all insurance plans cover mental health services. To find out what services are covered, please contact your insurance provider. 

Mr. Harman can accept out-of-network reimbursement for many insurance plans. If you expect to have your insurance cover the services provided by Mr. Harman, be sure to have your insurance information available and check with Mr. Harman to confirm coverage prior to setting up your first appointment. Otherwise, payment will have to be made by you at the time of service.

Please check your coverage carefully by asking the following questions:
  • Do I have mental health benefits?
  • What is my deductible and has it been met?
  • How many sessions per calendar year does my plan cover?
  • How much does my plan cover for an out-of-network provider?
  • What is the coverage amount per therapy session?
  • Is approval required from my primary care physician?
Services may be covered in full or in part by other health insurance or employee benefit plans. Upon request, documention that includes the necessary diagnostic and service codes can be provided to your insurance company. Your insurance company will make a determination as to any reimbursement you may be entitled to receive in accordance with your plan.

Cancellation Policy
In the event that you need to cancel or reschedule your appointment, you must do so at least 24 hours in advance of your scheduled appointment time. This advance notice allows your alloted time to be filled by another patient.

In the event that you fail to provide at least 24 notice, please be aware that you may be billed the full amount for your missed session.
